  • In order to increase your chances of being successful at achieving your goals, a certain protocol should be followed. Please ensure all your goals are 'SMART'.   • PERSONAL TRAINING INFORMED CONSENT, WAIVER AND RELEASE

  • I do hereby consent to participate in a personal training program that will include weight training and/or cardiovascular exercise. Because physical exercise can be strenuous and subject to risk of serious injury (including but not limited to musculoskeletal injury, spinal injuries, abnormal blood pressure responses, and rare instances heart attack or death), it is urged that you obtain physical examination from a doctor before using any exercise equipment or participating in any exercise activity. You agree that by participating in physical exercise or training activities, you do so entirely at your own risk.

    Any recommendation for changes in diet including the use of food supplements, weight reduction and/or body building enhancement products are your responsibility and you should consult a physician prior to undergoingany dietary or food supplement changes. You agree that you are voluntarily participating in these activities and use of these facilities and premises and assume all risks of injury, illness, or death. Fortitude Health and Training, LLC is not responsible for any loss of your personal property.

    <>This waiver and release of liability includes, without limitations, all injuries which may occur as a result of; a) your use of all amenities and equipment at Fortitude Health and Training, LLC facility and your participation in any activity, class, program, personal training, supervision, or instruction, b) the sudden and unforeseen malfunctioning of any equipment c) our instruction, training, supervision, or dietary recommendations and d) your slipping and/or falling while in Fortitude Health and Training, LLC on the premises, including adjacent sidewalks and parking areas.

    You acknowledge that you have carefully read this “waiver and release” and fully understand that it is release of liability. You expressly agree to release and discharge the trainer/instructor from any and all claims or causes of action and you agree to voluntarily give up or waive any right that you may otherwise have to bring a legal action against the Fortitude Health and Training, LLC for personal injury or property damage. To the extent that statute or case law does not prohibit releases for negligence, this release is also for negligence.

    If any portion of this release from liability shall be deemed by a Court of competent jurisdiction to be invalid, then the remainder of this release from liability shall remain in full force and effect and the offending provision or provisions severed here from.

    By signing this release, I acknowledge that I understand its content and that this release cannot be modified orally.

  • Please be on time for appointments. Sessions that start late still end at the scheduled time. If you are more than 10 minutes late and you have not contacted your trainer it will be considered a no-show and the client will be charged for that session.

    Cancellations need to be made at least 24 hours in advance of the scheduled training session.

    For no shows and cancellations less than 24 hours in advance the client will be charged for that session.

    Sessions are non-refundable, non-transeferable and expire within four months. Tandem trainings must also be cancelled or rescheduled at least 24 hours in advance. If one partner does not show for scheduled training they will be charged for that session.

  • Fortitude Health + Training Policies

  • In order to keep our membership prices low and in order to keep our practices fair to all of our members, Fortitude Health and Training has adopted the following policies. Initial each of the following:
    • While Fortitude's memberships do not include an initiation fee or a termination fee we will unilaterally require:
    • In person with Danielle Perreault or with Lisa Maria-Booth, a thirty day written notice is required in order to discontinue monthly auto pay.
    • Thirty dollar fee in order to reinstate a previously cancelled auto pay membership, sixty dollars for couples memberships.
    • Membership suspensions will be granted only in the case of injury or illness which specifically prohibits member from exercise and will be granted upon receipt of a letter confirming such by a doctor specializing in such illness or injury. Three month membership suspension for maternity leave; client may determine start time.
    • Medical membership suspensions shall be granted for up to and no longer than two calendar months at which point auto pay will resume.
    • From time to time credit and debit cards expire, need to be replaced or payments are unable to go through It is members responsibility to provide new and valid numbers in advance of monthly draft. A ten dollar fee will be assessed when Fortitude is unable to run of monthly auto pay.If account is not in good standing within 10 days of a failed monthly draft, the account will be sent to a collections agency for recovery of amounts currently owed and any additional amount owed on the account plus any fees charged by the collections agency.
    • A ten dollar fee will be assessed for each attempt to rerun cards which were not accepted on first attempt for monthly auto pay.
    • Couples memberships are available to two people living in same household with proof of residence.
    • Student memberships & drop in rates are available to full time students with proof of enrollment.
    • Open gym is available during scheduled times.
    • Memberships are non-transferable.
    • Fortitude Health and Training has a strict shoe policy to which we allow no exceptions. Outside shoes must be removed immediately upon entry to gym and left in black shelves to right of door. Exercise shoes must be shoes that are never worn outside; completely clean. Exercise shoes may be left at The Fort in the bins provided for your convenience. Bootcamp classes will go outside from time to time when the weather is forty degrees or higher. It is our members' responsibility to have outside shoes ready for those classes; they must be removed upon re entry into the gym. At The Fort we exercise down on the mats. It is members' responsibility to keep floors clean for fellow members. A $20 fee may be assessed when outside shoes are worn in The Fort.
